Does it still do this when the car/engine is warmed up (i.e. running for ~5-10 minutes)?

Check the Seafoam videos on Youtube. if you did not use seafoam then check the color of your smoke. if it is blue then your engine is gone. If white then something else.

What color is the smoke? On my first viewing I thought it was white, but on a second viewing, it looks grayish. The color of the smoke can tell you a lot:
