Manual transmission grinding

hi I have a manual trans 2008 corolla s with over 200k miles.

I bought it with 127k and I assume the clutch was replaced shortly before I bought it.

There’s been a grinding sound that started up while driving that’s gotten worse and worse. It’s in all gears but not when in neutral but it’s all the time with variable grinding noises while driving. I’ve changed the fluid a few times. Would it be a bearing in the clutch that would cause this or an internal issue in the transmission?

Any feedback would be greatly appreciated

I’ve had the car go into limp mode completely losing throttle. No codes. I was assuming the sealed throttle body or maybe my clutch causing the issues?
